Parapharyngeal emphysema after blast injury of the oral cavity is very rare. It occurs as a consequence of sudden increase in the air pressure in the oropharynx which lacerates the buccogingival mucosa and enables entering of the air into the neck spaces. A possible mechanism of development and the way of penetration of the air into the parapharynx are explained.
