Diffuse lung diseases are difficult to diagnose because of its vast etiology, clinical sintoms and the frequent inconclusive complementary exams making diagnostic thorachotomy a necessary procedure. Objective: to compare the diagnosis based on clinical exams and the histological result of the surgical biopsy (SB). Population: 98 patients were retrospectively reviewed, 51 female and 47 male with a median age of 55,4 years, who underwent SB for diffuse lung disease during 2003-2004. Results: There was no mortality or postoperative complications. The average admittance time was 4 days. Discussion: SB confirmed the initial diagnosis in 55(56,1%) patients. In the other 27 (27,5%) patients SB allowed a new diagnosis in up to 50% in some lung diseases. Conclusion: this series results and the diagnostic changes produced makes SB a fundamental procedure in the diagnostic evaluation of diffuse lung diseases.
