    The prolonged debate on the safety and use of Depo-Provera appears to be moving to a decision stage. Depo-Provera has been approved for use under certain conditions in Sweden, but the Swedish International Development Authority recently decided to discontinue supplying or funding the purchase of Depo as part of its international aid program because the agency would not be able to monitor longterm adherence to the Swedish health board's recommendations and it was felt that more research was needed on infant exposure to Depo. Depo is approved in the UK for short-term but not longterm contraceptive use. The refusal represents the 1st time the recommendation of the Committee on Safety of Medicine has been overruled, in this case by a minister without medical background who based his decision on nonmedical reasons. In the US, a Food and Drug Administration Public Board of Inquiry will be held in January 1983 to review Depo's status. In Canada a Special Advisory Committee on Reproductive Physiology reported that Depo-Provera does not present any undue health hazard and that it may be used as a contraceptive. Depo is currently approved for longterm contraceptive use in approximately 80 countries, including 10 developed countries. The International Planned Parenthood Federation's Medical Advisory Panel believes that Depo is safe for use in all countries. Although no studies have systematically followed the health and development of infants exposed in utero to Depo, any teratogenic risk is believed to be small. Studies of the effect of Depo on lactation and on the breastfed infant demonstrate that Depo does not decrease and may increase milk production, and that only trace levels of ingested medroxyprogesterone acetate are present in the circulatory systems of the nursing infants. A Bangladesh study found that maternal contraceptive use of any kind did not influence the growth of breast fed children. A 2-year, controlled, crossover trial involving 23 female homozygous sickle cell anemia patients in Jamaica who received 50 mg of Depo-Provera every 3 months indicated that Depo may have beneficial effects on sickle cell disease.
