    Cancer- and chemotherapy-induced anemia is a common complication that arises in virtually all oncology branches. Darbepoetin alfa (Aranesp(R)), a novel erythropoiesis-stimulating protein, is a drug appropriate for treatment of anemia and chemotherapy-induced anemia in cancer patients. Aim: In order to audit the clinical use of darbepoetin alfa in oncology practice in Spain, we conducted a questionnaire-based analysis of darbepoetin alfa at a fixed dose of 150 mcg s.c. once weekly in patients who were receiving chemotherapy with or without other anti-cancer therapies. Design: Cohort-based retrospective study involving patients from 40 Spanish health centers. Patients: The records of 666 patients with median age of 60.37 (range: 16-100 years) of both sexes (men 41%; women 59%) were included in this study. The most frequent types of tumors were: breast (20%), ovarian (15.2%), lung (14.5%), colorectal (10.5%) hematologic (6.8%), and head and neck (6.2%), with a higher percentage of stage IV disease (60.6%). Results: Most patients were treated with platinum-based chemotherapy (35.7%), followed by taxanes (14.0%), combinations of platinum and taxanes (19.6%) or other therapies (30.7%). About 62.6% of patients received first-line chemotherapy, while 21.9% and 15.5% required second-line and third-line chemotherapy, respectively. The baseline hemoglobin concentration was <9 g/dl in 27.5% of the patients, between 9 and 10 g/dl in 36%, and >/=10 g/dl in 36.5% (including 5.1% of patients with hemoglobin >/=11 g/dl). The average treatment period with darbepoetin alfa was 8.9 weeks. The dose of the drug was increased at week 4 in 17.7% of the patients. At the end of the treatment, 83.8% of the patients had hemoglobin levels >/=10 g/dl and 56.5% were at >/=11 g/dl. Overall, hematopoietic response to darbepoetin alfa was unrelated tumor type, stage of disease, regimen or line of chemotherapy, or radiotherapy. The most relevant factors of choice for darbepoetin alfa were less frequent administration (84% of cases), rapid response (37%) and high efficacy (28%). Conclusions: Darbepoetin alfa 150 microg once weekly used for amelioration of chemotherapy-induced anemia effectively maintained hemoglobin levels in most patients within the daily oncology routine. Most of the oncologists participating in the study confirmed the willingness to prescribe darbepoetin alfa based on the convenience of a less frequent administration regimen. (c) 2006 Prous Science.
