    Recent data suggest that estrogen receptors are not only useful in predicting the response to endocrine treamtent of advanced breast cancer, but also in predicting the effect of chemotherapy in such cancer (Lippman, 1978). Jensen (1978) and Knight et al (1977) observed that patients with no or very low estrogen-receptor content in their tumors have a shorter disease-free interval. Another important clinical parameter for response to endocrine manipulation is the site of metastases. Patients with visceral metastases, especially of liver and brain, are known to respond poorly to any kind of endocrine therapy. Duration of menopause is another clinical parameter that must be considered. Patients in the 1st 5 years after menopause respond very poorly to ablative and additive endocrine treatments. Women in the early years of menopause have been found to have quantitative ER (estrogen receptor) contents comparable to those of elderly patients. Some patients are not suitable for endocrine treatment only regardless of their positive receptor status; such patients are candidates for a combined hormonal and cytotoxic therapy. If ER (+) patients with unfavorable prognosis are excluded from trials, better correlations in the remaining subjects may be achieved.
